The evolution isn’t just organizational growth. It reflects a deliberate strategic choice: to expand from a population-specific provider into a full healthcare system capable of serving anyone who faces barriers to care.
The Federally Qualified Health Center Model
Equitas operates as a Federally Qualified Community Health Center look-alike, a designation that enables it to offer services on a sliding fee scale based on income rather than at fixed market rates. This structure is what makes healthcare access real rather than aspirational for patients without insurance or with limited financial resources.
The sliding fee model means a patient who makes $15,000 a year doesn’t pay the same as a patient who makes $80,000 a year. That’s a deliberate design choice embedded in the organization’s operating model — not a fundraising campaign or a charity initiative.
17 locations across 11 Ohio cities, plus mobile health units, extend the reach into communities where residents couldn’t otherwise travel to access care. The mobile outreach van in Cincinnati and surrounding areas provides HIV and STI testing, connection to long-term care resources, and assistance with medical insurance enrollment, meeting people where they are rather than requiring them to navigate a healthcare system that has historically been hostile to them.
The Social Enterprise Pharmacy Model
Equitas operates its community pharmacies as social enterprises, investing 100% of pharmacy profits back into the organization’s programs and services. This is a structural choice that removes the profit extraction typical of retail pharmacy and redirects it entirely toward mission delivery.
Equitas Health Pharmacy & Priz provides access to medications and supplies to patients of all income levels, including those who would otherwise struggle to afford their HIV regimens, PrEP, and other specialty medications. For patients managing chronic conditions, medication access is not peripheral to care. It’s the most critical component.
The pharmacy social enterprise model is worth noting for social entrepreneurs: running a commercial operation alongside a social mission, with all commercial proceeds reinvested into the mission, is one of the more durable structures available. It generates operational independence from donor cycles while remaining mission-aligned.
The healthcare system has historically treated LGBTQ+ patients as problems to manage rather than people to serve. Equitas was built on the opposite premise — that affirming, competent care is both a moral baseline and a better clinical model.
Addressing Social Determinants at Scale
In 2016, Equitas implemented PRAPARE, a national social determinants of health assessment tool, to systematically track the factors beyond clinical care that drive health outcomes: housing stability, food access, transportation, employment, safety.
This data collection enables targeted referrals and intervention rather than treating patients as clinical cases disconnected from their social context. A patient whose HIV regimen is failing because they’re experiencing food insecurity needs different support than one whose barriers are primarily logistical.
The mobile sexual health clinic expanding into southwest Ohio extends this whole-person approach geographically, reaching LGBTQ+ populations in counties where competent, affirming providers are effectively unavailable.
